Cilofexor + selonsertib is an investigational oral combination therapy for advanced liver fibrosis due to nonalcoholic steatohepatitis (NASH). Cilofexor is a nonsteroidal farnesoid X receptor (FXR) agonist that modulates bile acid metabolism and reduces hepatic inflammation and fibrosis. Selonsertib is a small molecule inhibitor of apoptosis signal-regulating kinase 1 (ASK1), which plays a role in promoting inflammation, apoptosis, and fibrosis under oxidative stress conditions. The combination aims to target distinct pathogenic pathways in NASH by reducing hepatic steatosis, inflammation, and fibrogenesis. Both agents are being developed by Gilead Sciences for use alone or in various combinations for the treatment of NASH with advanced fibrosis[4][6][7].
