Overview

This is a combination of cimicifuga racemosa (commonly known as black cohosh), a herbal supplement often used to alleviate menopausal symptoms, and tamoxifen, a selective estrogen receptor modulator (SERM) widely used in the treatment and prevention of estrogen receptor-positive breast cancer. Black cohosh is believed to exert its effects through serotonergic pathways or other non-estrogenic mechanisms, though its precise mechanism remains unclear. Tamoxifen acts primarily as an antagonist of the estrogen receptor in breast tissue, inhibiting tumor growth by blocking estrogen signaling. The combination has been studied for relief of vasomotor and psychovegetative symptoms (such as hot flashes and sleep disturbances) induced by tamoxifen therapy in breast cancer patients[1][3][8]. Some clinical studies report improvement in these symptoms with good tolerability[1][8], but laboratory research indicates that black cohosh may inhibit cytochrome P450 enzymes CYP2D6 and CYP3A4—key enzymes responsible for converting tamoxifen into its active metabolites—potentially reducing the efficacy of tamoxifen[2][5][6]. No consistent serious adverse events have been reported with this combination; however, due to possible drug-herb interactions affecting metabolism and efficacy, caution is advised.
