**CIN-107 + metformin** is a combination of two pharmaceutical agents: CIN-107 (baxdrostat), a highly potent and selective oral small molecule inhibitor of human aldosterone synthase (CYP11B2), and metformin, a well-established antihyperglycemic agent of the biguanide class. CIN-107 reduces aldosterone synthesis, resulting in decreased sodium retention and blood pressure, without affecting cortisol levels[1][3][5]. Metformin acts by decreasing hepatic glucose production, suppressing gastrointestinal absorption of glucose, and improving peripheral insulin sensitivity[2][5]. Combination therapy is investigated mainly for patients with hypertension and/or type 2 diabetes, with pharmacokinetic studies confirming no impact of CIN-107 on systemic exposure to metformin and no clinically significant pharmacodynamic interactions[1][4][5]. The combination is particularly relevant in subjects with hypertension and comorbid diabetes due to potential additive/synergistic effects on blood pressure and metabolic control.
