Cinrebafusp alfa + ramucirumab + paclitaxel is a combination regimen under investigation for the treatment of HER2-expressing gastric and gastroesophageal junction cancers. Cinrebafusp alfa (PRS-343) is a bispecific fusion protein that targets both 4-1BB (CD137), a T-cell co-stimulatory receptor, and HER2, an oncogenic receptor overexpressed in certain tumors. This dual targeting aims to enhance anti-tumor immune responses by activating T cells specifically at the tumor site. Ramucirumab is a monoclonal antibody that inhibits angiogenesis by antagonizing vascular endothelial growth factor receptor 2 (VEGFR2). Paclitaxel is a microtubule inhibitor that disrupts cell division. The combination leverages immune activation, inhibition of tumor angiogenesis, and direct cytotoxicity to cancer cells. The regimen is currently being evaluated in phase 2 clinical trials for patients with HER2-high gastric cancer[1][2][3][4][5][8].
