This is a combination drug consisting of ciprofloxacin, dexamethasone, and N-acetyl cysteine. Ciprofloxacin is a fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, leading to the inhibition of bacterial replication and cell death. Dexamethasone is a synthetic glucocorticoid corticosteroid with potent anti-inflammatory properties; it reduces inflammation by suppressing multiple inflammatory cytokines and mediators. N-acetyl cysteine (NAC) acts as an antioxidant and mucolytic agent; it replenishes intracellular glutathione levels, scavenges reactive oxygen species, disrupts biofilms, and has demonstrated synergistic effects with antibiotics against resistant bacteria. This combination has been studied primarily for topical use in refractory ear infections (chronic suppurative otitis media or persistent otorrhea), where NAC augments the efficacy of ciprofloxacin/dexamethasone by enhancing antibacterial activity—especially against biofilm-forming pathogens such as Pseudomonas aeruginosa—and providing additional anti-inflammatory protection[1][2][6].
