Ciprofloxacin + metronidazole is a combination antibiotic regimen being investigated by Sunnybrook Health Sciences Centre in the PARFAIT (Perianal Abscess Recurrence and Fistula Formation: Antibiotics Following Incision and Drainage) trial. The regimen consists of ciprofloxacin, a fluoroquinolone that inhibits bacterial DNA gyrase and topoisomerase IV, and metronidazole, a nitroimidazole that causes DNA strand breakage in anaerobic bacteria. This combination provides broad-spectrum coverage against gram-positive, gram-negative, and anaerobic pathogens. It is being evaluated as a post-surgical treatment following the incision and drainage of perianal abscesses to reduce the incidence of perianal fistula formation and abscess recurrence.
