Overview
Ciprofloxacin + minocycline + metronidazole, commonly known as Triple Antibiotic Paste (TAP) or 3Mix-MP, is a multi-antibiotic topical paste used as an intracanal medicament in endodontics. It combines three distinct antimicrobial agents: ciprofloxacin (a fluoroquinolone), minocycline (a tetracycline), and metronidazole (a nitroimidazole), often formulated with a macrogol/propylene glycol (MP) carrier to enhance dentinal tubule penetration. This combination targets the polymicrobial bacterial communities (both aerobic and anaerobic) responsible for persistent root canal infections, working by disrupting biofilms and eradicating persistent pathogens. It is primarily used to treat endodontic infections, including symptomatic apical periodontitis and necrotic pulp conditions, and has been evaluated in clinical trials (such as those sponsored by Qassim University) for reducing postoperative pain following root canal preparation. Known limitations include potential tooth discoloration due to minocycline and cytotoxicity at high concentrations.
