Overview

This is a combination of three antibiotics—ciprofloxacin, trimethoprim, and sulfamethoxazole. Ciprofloxacin is a fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, thereby preventing DNA replication and transcription in susceptible bacteria. Trimethoprim is an antifolate agent that inhibits dihydrofolate reductase, blocking the synthesis of tetrahydrofolic acid required for bacterial DNA production. Sulfamethoxazole is a sulfonamide antibiotic that interferes with folic acid synthesis by inhibiting dihydropteroate synthase. The combination of trimethoprim and sulfamethoxazole (often known as co-trimoxazole) provides synergistic inhibition of sequential steps in the folate pathway, while ciprofloxacin adds an additional mechanism targeting bacterial DNA processes. This triple combination may be used off-label or in research settings for prophylaxis or treatment of complex or resistant infections where dual therapy has failed or resistance patterns warrant broader coverage[3][6][7].
