Cisplatin delivered via isolated lung suffusion (also known as pulmonary suffusion) is a regional chemotherapy delivery technique developed at Roswell Park Cancer Institute for the treatment of primary and metastatic lung tumors. The procedure involves a minimally invasive thoracoscopic approach to temporarily isolate the pulmonary circulation of the affected lung, allowing for the infusion of high-dose cisplatin directly into the pulmonary artery. The drug is allowed to dwell for 30 minutes before being aspirated, after which normal blood flow is restored. This targeted approach achieves local drug concentrations up to 20-fold higher than those possible with systemic administration while significantly reducing systemic toxicity. Cisplatin is a platinum-based small molecule that functions by forming covalent adducts and crosslinks with DNA, which inhibits DNA replication and transcription, ultimately triggering apoptosis in rapidly dividing cancer cells.
