This combination regimen is a multi-agent chemo-immunotherapy protocol being investigated by ImmunityBio for the first-line treatment of advanced or metastatic non-small cell lung cancer (NSCLC). It integrates the IL-15 superagonist fusion protein nogapendekin alfa inbakicept (NAI; Anktiva) with the PD-1 checkpoint inhibitor pembrolizumab and standard-of-care platinum-based chemotherapy (pemetrexed combined with either cisplatin or carboplatin). Nogapendekin alfa inbakicept is a first-in-class IL-15 receptor agonist complex designed to activate and expand CD8+ T cells and natural killer (NK) cells, mimicking the natural process of IL-15 transpresentation. Pembrolizumab works by blocking the interaction between PD-1 and its ligands, thereby restoring anti-tumor T-cell activity. The chemotherapy components provide direct cytotoxic effects by inducing DNA cross-linking (cisplatin/carboplatin) and inhibiting essential enzymes in the folate pathway (pemetrexed), which may also enhance the immunogenicity of the tumor. This five-drug regimen is currently being evaluated in the Phase 3 QUILT-2.023 clinical trial.
