Overview

This is a combination regimen consisting of cisplatin, custirsen, and gemcitabine. - **Cisplatin** is a platinum-based chemotherapeutic agent that forms DNA crosslinks and inhibits DNA synthesis and function, leading to apoptosis in rapidly dividing cells. - **Gemcitabine** is a nucleoside analog (antimetabolite) that incorporates into DNA during replication, causing chain termination and cell death. It also inhibits ribonucleotide reductase, further depleting deoxynucleotide pools required for DNA synthesis[7][10]. - **Custirsen** (OGX-011) is an antisense oligonucleotide targeting clusterin mRNA. Clusterin is an antiapoptotic protein upregulated in response to cellular stress and cancer therapies; by inhibiting its production, custirsen sensitizes tumor cells to chemotherapy-induced apoptosis[3]. The combination has been studied primarily in advanced non-small cell lung cancer (NSCLC), where the addition of custirsen to standard gemcitabine/cisplatin regimens was evaluated for improved efficacy by reducing resistance mechanisms mediated by clusterin[3].
