The combination of cisplatin, dalotuzumab, and pemetrexed is an investigational treatment regimen primarily studied for non-small cell lung cancer (NSCLC), particularly non-squamous NSCLC. This combination brings together three distinct agents with complementary mechanisms of action. Dalotuzumab (MK-0646) is a humanized IgG1 monoclonal antibody targeting the Insulin-like Growth Factor 1 Receptor (IGF-1R), aiming to block proliferation and survival signals. Pemetrexed is an antifolate antineoplastic agent disrupting folate-dependent metabolic processes, approved for NSCLC and malignant pleural mesothelioma. Cisplatin is a platinum-based chemotherapy commonly used with pemetrexed in NSCLC. A randomized phase II trial investigating this combination as first-line therapy for non-squamous lung cancer was initiated but closed prematurely due to a sponsoring company's decision and dalotuzumab's overall failure to demonstrate sufficient efficacy in clinical development.
