Overview

This is a multi-agent chemotherapy combination consisting of three cytotoxic drugs: cisplatin, doxorubicin, and mitomycin. Each drug has a distinct mechanism of action: - **Cisplatin** is an alkylating-like agent that forms DNA crosslinks leading to apoptosis. - **Doxorubicin** is an anthracycline antibiotic that intercalates into DNA and inhibits topoisomerase II. - **Mitomycin** (commonly administered as mitomycin C) is an alkylating agent that crosslinks DNA. The combination has been studied under various acronyms such as PAM (cisplatin/doxorubicin/mitomycin) for advanced gastric carcinoma[1], MAP for metastatic nasopharyngeal carcinoma induction therapy[2][5], and MDP or similar regimens for non-muscle-invasive bladder cancer via intravesical administration[7]. It has also been evaluated preoperatively in soft tissue sarcomas with chemoradiation protocols[4]. The rationale behind combining these agents lies in their complementary mechanisms to maximize tumor cell kill while attempting to minimize overlapping toxicities. The main toxicities are myelosuppression and organ-specific side effects from each component.
