Overview

Cisplatin + etoposide + bleomycin is a combination chemotherapy regimen commonly referred to as "BEP." It consists of three cytotoxic agents: - **Cisplatin** is a platinum-based compound that forms DNA crosslinks, leading to apoptosis in rapidly dividing cells. - **Etoposide** inhibits topoisomerase II, causing DNA strand breaks and preventing cell division. - **Bleomycin** induces single- and double-strand breaks in DNA through free radical formation. This combination exploits different mechanisms of action to maximize cancer cell kill. BEP is primarily used as first-line therapy for malignant germ cell tumors (testicular and ovarian), but may also be used for other cancers. Each drug in the combination is FDA-approved for cancer treatment[4][5]. The regimen has high cure rates but carries risks of significant toxicities including pulmonary toxicity (from bleomycin), nephrotoxicity (from cisplatin), myelosuppression, infertility, ototoxicity, neuropathy, secondary malignancies (notably acute myeloid leukemia from etoposide), and cardiovascular complications[2][3][5].
