Cisplatin + fotemustine is a combination chemotherapy regimen consisting of two small molecule cytotoxic agents. Cisplatin is a platinum-based compound that forms DNA crosslinks, leading to apoptosis in rapidly dividing cells. Fotemustine is a nitrosourea alkylating agent that also induces DNA damage by forming interstrand crosslinks and carbamoylation of proteins, with notable activity against metastatic melanoma (especially brain metastases) and poor prognosis lung cancers. The combination has been investigated primarily for advanced or metastatic non-small cell lung cancer and malignant melanoma, including cases with brain metastases. Clinical studies have shown partial response rates in these populations; toxicity is generally mild but includes cumulative nephrotoxicity and peripheral neuropathy from cisplatin[1][2][4].
