Overview

This is a combination chemotherapy regimen consisting of three agents: - **Cisplatin** is a platinum-based chemotherapeutic that forms DNA crosslinks, leading to apoptosis in rapidly dividing cells. - **Gemcitabine** is a nucleoside analog that inhibits DNA synthesis by incorporation into DNA and inhibition of ribonucleotide reductase. - **Iniparib** was initially thought to be a poly(ADP-ribose) polymerase (PARP) inhibitor but later shown to act via non-selective protein modification through cysteine adducts. Iniparib induces DNA damage (as indicated by gamma-H2AX), causes cell cycle arrest at the G2/M phase, and potentiates the effects of other DNA-damaging agents[1][5][7]. This combination has been studied primarily in metastatic non-small-cell lung cancer (NSCLC)[1] and triple-negative breast cancer, as well as explored for ovarian cancer with platinum-sensitive or -resistant disease[2]. The addition of iniparib to gemcitabine/cisplatin did not show significant improvement over standard regimens but maintained a comparable safety profile[1].
