Citicoline + memantine is a fixed-dose combination of two small molecule drugs used primarily in the management of cognitive impairment, particularly in Alzheimer's disease and mixed dementia. Citicoline (cytidine-5'-diphosphocholine) acts as a neuroprotective agent and cholinergic precursor, enhancing acetylcholine synthesis, increasing phosphatidylcholine and other membrane phospholipids, boosting neurotransmitter levels (acetylcholine, dopamine, noradrenaline), reducing free radical generation at ischemic sites, inhibiting apoptosis, and supporting mitochondrial energy metabolism. Memantine is an uncompetitive antagonist of the N-methyl-D-aspartate (NMDA) receptor subtype of glutamate receptors; it reduces pathological glutamatergic excitotoxicity implicated in neurodegeneration while sparing physiological synaptic transmission. The combination has been shown to be more effective than memantine alone or memantine plus acetylcholinesterase inhibitors in maintaining or improving cognitive function over 6–12 months in patients with Alzheimer's disease or mixed dementia[1][2][4]. Both agents are generally well tolerated.
