**Citric acid dialysate + heparin** is a combination therapy used during hemodialysis. Citric acid dialysate, typified by products such as Citrasate, replaces conventional acetic acid dialysate and provides anticoagulant activity within the dialysis circuit due to its calcium-chelating properties. Heparin is a well-established systemic anticoagulant used to prevent clotting in the extracorporeal circuit. The combination is intended to reduce the total dose of heparin required for adequate anticoagulation by leveraging the local anticoagulant effect of citrate, which is generally limited to the circuit and does not affect systemic coagulation as strongly as heparin. The dialysate is typically used in chronic or extended duration hemodialysis, and clinical studies have shown that it allows significant reduction in heparin dosing—maintaining dialysis adequacy and reducing risks such as bleeding or heparin-induced thrombocytopenia. Citrate dialysate also modifies mineral metabolism and improves hemodynamic stability during dialysis[1][3][5][6][7].
