Overview

CALGE-VEN- RIC (more commonly referred to as CLAGE-VEN-RIC) is a sequential combination chemotherapy and reduced-intensity conditioning (RIC) regimen developed for the treatment of refractory acute myeloid leukemia (AML). The protocol involves a debulking phase consisting of the CLAGE-VEN regimen—comprising cladribine (a purine nucleoside analog), cytarabine (an antimetabolite), etoposide (a topoisomerase II inhibitor), and venetoclax (a BCL-2 inhibitor). This is followed by a reduced-intensity conditioning phase, typically consisting of fludarabine, busulfan, and melphalan (Flu-Bu-Mel) or total marrow irradiation (TMI), prior to allogeneic hematopoietic stem cell transplantation. The regimen is designed to improve remission rates in patients with highly resistant AML by combining traditional cytotoxic chemotherapy with targeted anti-apoptotic inhibition.
