This is a multi-agent chemotherapy regimen composed of five drugs—cladribine, cytarabine, tretinoin (all-trans retinoic acid), filgrastim (granulocyte colony-stimulating factor), and idarubicin. Each component has a distinct mechanism of action: - Cladribine is a purine nucleoside analog that inhibits DNA synthesis and repair, leading to cell death in rapidly dividing cells. - Cytarabine is an antimetabolite that interferes with DNA synthesis by acting as a cytosine analog. - Tretinoin induces differentiation of promyelocytic leukemia cells by activating retinoic acid receptors. - Filgrastim stimulates the production and maturation of neutrophils via activation of the granulocyte colony-stimulating factor receptor. - Idarubicin is an anthracycline antibiotic that intercalates into DNA, inhibiting topoisomerase II and generating free radicals to induce apoptosis in cancer cells. This combination targets multiple pathways involved in leukemic cell proliferation, survival, and differentiation. It has been studied primarily for acute myeloid leukemia (AML) or high-risk myelodysplastic syndrome (MDS), especially in relapsed/refractory settings or high-risk patients[2][4][7][9]. The regimen aims to maximize anti-leukemic efficacy through synergistic mechanisms while supporting hematopoietic recovery with filgrastim.
