Overview

CLAM + PBSC is a salvage chemotherapy regimen followed by peripheral blood stem cell (PBSC) transplantation, primarily utilized in the treatment of relapsed or refractory acute myeloid leukemia (AML). The chemotherapy component, CLAM, consists of cladribine, leucovorin, cytarabine (Ara-C), and mitoxantrone. Cladribine, a purine nucleoside analog, acts as a biochemical modulator that enhances the intracellular accumulation of cytarabine triphosphate (Ara-CTP), the active metabolite of cytarabine, thereby increasing its cytotoxic efficacy. Mitoxantrone, a type II topoisomerase inhibitor and DNA intercalator, provides additional antineoplastic activity. Leucovorin is included to modulate nucleoside metabolism and potentially enhance the effects of the pyrimidine analogs. Following the intensive chemotherapy, peripheral blood stem cells are infused to restore hematopoietic function and facilitate bone marrow recovery.
