Clarithromycin + lenalidomide is an oral combination regimen of two small molecule drugs, clarithromycin (a macrolide antibiotic) and lenalidomide (an immunomodulatory agent). This combination has been investigated primarily for hematologic malignancies, including relapsed/refractory extranodal marginal zone lymphoma (MZL) and multiple myeloma. Clarithromycin exhibits antineoplastic activity in some lymphomas and can enhance the efficacy of lenalidomide by overcoming drug resistance mechanisms. Lenalidomide acts as an immunomodulator with antiangiogenic and direct tumoricidal effects. The combination is generally well-tolerated, with a safety profile comparable to other approved oral regimens for these indications[4].
