Overview

This is a fixed-duration combination therapy consisting of three drugs—clarithromycin (a macrolide antibiotic), naproxen (a nonsteroidal anti-inflammatory drug), and oseltamivir (a neuraminidase inhibitor antiviral)—used primarily in the treatment of hospitalized patients with severe influenza, particularly those with pneumonia or significant respiratory compromise. The regimen typically involves clarithromycin 500 mg, naproxen 200 mg, and oseltamivir 75 mg administered twice daily for two days, followed by continued oseltamivir monotherapy. The rationale behind this combination is to provide both direct antiviral activity against influenza virus (via oseltamivir) and immunomodulatory/anti-inflammatory effects (via clarithromycin and naproxen). Clinical trials have shown that this combination can reduce mortality rates at 30 and 90 days, shorten hospital stays, accelerate fever resolution, decrease viral titers more rapidly than oseltamivir alone, and lower the severity of pneumonia in hospitalized patients[1][2][4]. Adverse events are uncommon and similar to those seen with standard care.
