Clarithromycin + zidovudine is a combination of two small molecule pharmaceuticals used for the treatment or prevention of infections and HIV/AIDS. Clarithromycin is a macrolide antibiotic primarily used to treat bacterial infections such as those caused by *Mycobacterium avium* complex in immunocompromised individuals. Its mechanism involves inhibition of bacterial protein synthesis by binding to the 50S ribosomal subunit. Zidovudine is a nucleoside reverse transcriptase inhibitor (NRTI) used as an antiretroviral agent in the management of HIV infection. Zidovudine inhibits the viral reverse transcriptase enzyme, preventing viral DNA synthesis. When administered together, clarithromycin can reduce the serum concentrations of zidovudine by interfering with its gastrointestinal absorption. This interaction can be minimized by separating the dosing of the two agents by at least two hours. This combination is not a standard fixed-dose drug product but may be co-administered in relevant clinical scenarios, particularly among patients with advanced HIV, to address both HIV infection and opportunistic bacterial infections[1][2][4].
