Overview

Clesacostat + ervogastat is an investigational combination therapy developed by Pfizer for the treatment of non-alcoholic fatty liver disease (NAFLD) and non-alcoholic steatohepatitis (NASH) with liver fibrosis. The combination consists of clesacostat (PF-05221304), a liver-targeted inhibitor of acetyl-CoA carboxylase (ACC1 and ACC2), and ervogastat (PF-06865571), a diacylglycerol O-acyltransferase 2 (DGAT2) inhibitor. Clesacostat works by inhibiting the first committed step of de novo lipogenesis, while ervogastat targets the final step of triglyceride synthesis. By combining these two mechanisms, the therapy aims to significantly reduce hepatic fat content. Furthermore, the addition of a DGAT2 inhibitor helps to mitigate the increase in serum triglycerides that is commonly observed with ACC inhibition alone. The combination has been granted Fast Track designation by the FDA for NASH with liver fibrosis and is currently undergoing Phase 2 clinical evaluation.
