clindamycin + ciprofloxacin + metronidazole is a combination antimicrobial formulation used primarily in endodontics, specifically for the disinfection of the root canal system during regenerative endodontic procedures (REP). This combination is a variation of the traditional 'Triple Antibiotic Paste' (TAP), where clindamycin replaces minocycline to prevent the permanent tooth discoloration often caused by the latter. Each component provides targeted activity: clindamycin (a lincosamide) inhibits protein synthesis in Gram-positive and anaerobic bacteria; ciprofloxacin (a fluoroquinolone) provides broad-spectrum bactericidal activity against Gram-negative organisms by inhibiting DNA replication; and metronidazole (a nitroimidazole) is specifically effective against obligate anaerobes. The paste is applied locally as an intracanal medicament to create a sterile environment that promotes pulp-dentin complex regeneration or revascularization in immature permanent teeth with necrotic pulp.
