This combination consists of four agents: **CLN-619**, a humanized IgG1 monoclonal antibody targeting MICA/B ligands, combined with **paclitaxel**, **carboplatin**, and **pemetrexed**. - **CLN-619** binds to the alpha-3 domain of MICA/B on tumor cells, preventing their proteolytic shedding, thus increasing ligand density at the tumor surface and enhancing natural killer (NK) cell-mediated cytotoxicity through simultaneous engagement of NKG2D and CD16A on immune effector cells[3][7]. - **Paclitaxel** stabilizes microtubules, inhibiting cell division and inducing apoptosis, and is classified as a taxane antineoplastic agent[2]. - **Carboplatin** is a platinum-based DNA-alkylating agent that induces apoptosis by cross-linking DNA[2]. - **Pemetrexed** is a multi-targeted antifolate that inhibits several enzymes involved in folate metabolism and DNA synthesis. This combination is under clinical investigation for the treatment of solid tumors, with particular expansion cohorts in endometrial cancer and non-small cell lung cancer (NSCLC)[7][9]. As of June 2024, there are no approved indications for this specific four-drug combination, but phase 1 studies have reported favorable safety and some objective clinical responses[7][9]. CLN-619 was developed by Cullinan Mica, while paclitaxel, carboplatin, and pemetrexed are generically available chemotherapeutic agents.
