CLN-619 + pembrolizumab

**CLN-619 + pembrolizumab** is a combination therapy under investigation for advanced solid tumors, especially those not typically responsive to immune checkpoint inhibitors. CLN-619 is a novel, humanized IgG1 monoclonal antibody that targets the MICA/B ligands on tumor cells, preventing their proteolytic shedding and enhancing NKG2D-dependent cytotoxicity by both innate (NK) and adaptive (T) immune cells. Pembrolizumab, known commercially as Keytruda, is an established PD-1 checkpoint inhibitor monoclonal antibody that boosts T-cell mediated anti-tumor activity. Early clinical results show that the combination leads to objective responses—including in traditionally checkpoint-inhibitor-resistant cancers such as those with EGFR or ALK-mutated NSCLC. The safety profile is favorable, with infusion-related reactions and fatigue as the most common adverse effects, most being grade 1/2.
