Clobetasol propionate + photobiomodulation is an investigational combination therapeutic regimen being studied for the management of oral lesions in patients with pemphigus vulgaris. The regimen combines clobetasol propionate 0.05% mouthwash, a high-potency topical corticosteroid, with photobiomodulation (PBM) therapy, a non-pharmacological modality using low-level laser or LED light. Clobetasol propionate exerts its effects by binding to glucocorticoid receptors, leading to the downregulation of pro-inflammatory cytokines and immunosuppression. Photobiomodulation targets mitochondrial chromophores, specifically cytochrome c oxidase, to enhance adenosine triphosphate (ATP) production, modulate oxidative stress, and accelerate the healing of recalcitrant oral erosions. This dual approach aims to provide rapid symptomatic relief and facilitate mucosal repair in autoimmune blistering diseases.
