Clodronic acid + vitamin D is a combination therapy being evaluated for the management of lateral fragility fractures of the femur, particularly in elderly patients with osteoporosis. Clodronic acid is a first-generation, non-nitrogen-containing bisphosphonate that binds to bone hydroxyapatite and is internalized by osteoclasts. Within the cell, it is metabolized into a non-hydrolyzable analog of adenosine triphosphate (ATP), which inhibits mitochondrial function and leads to osteoclast apoptosis, thereby reducing bone resorption. Vitamin D (typically cholecalciferol) is co-administered to ensure adequate calcium absorption and support bone mineralization. This specific combination is being studied by the Azienda Ospedaliero-Universitaria Consorziale Policlinico di Bari to evaluate its impact on biochemical bone markers and functional recovery (measured by the Harris Hip Score) following surgical stabilization of femur fractures with intramedullary nails.
