Clofarabine + busulfan is a combination chemotherapy regimen primarily used as a conditioning treatment before allogeneic hematopoietic stem cell transplantation, especially in patients with acute lymphoblastic leukemia (ALL) and high-risk myeloid leukemias[2][4]. Clofarabine is a second-generation purine nucleoside analog that interferes with DNA synthesis and repair by inhibiting DNA polymerase, causing DNA damage, and inducing apoptosis in cancer cells[5]. Busulfan is a bifunctional alkylating agent that forms carbonium ions after absorption; these ions alkylate DNA, leading to cross-linking and strand breaks that disrupt replication and transcription, ultimately resulting in cell death[1][3][8]. The combination of clofarabine (which impairs DNA repair) with busulfan (which causes extensive DNA cross-linking) results in synergistic cytotoxicity against leukemia cells by amplifying complex DNA damage beyond what either drug achieves alone[6].
