Clomiphene citrate + tamoxifen is a combination therapy consisting of two selective estrogen receptor modulators (SERMs). It is primarily investigated for ovulation induction in women with anovulatory infertility, such as those with polycystic ovary syndrome (PCOS). Clomiphene citrate acts as an estrogen antagonist in the hypothalamus, blocking negative feedback and stimulating the release of gonadotropin-releasing hormone (GnRH), which in turn increases follicle-stimulating hormone (FSH) and luteinizing hormone (LH) secretion. Tamoxifen acts through a similar mechanism but is often added to mitigate the anti-estrogenic effects of clomiphene on the endometrium, potentially improving endometrial thickness and pregnancy outcomes. This combination is often used in patients who have shown resistance to clomiphene monotherapy.
