Overview

This is a combination of three antibiotics—cloxacillin, cefazolin, and daptomycin—used for the treatment of severe Gram-positive bacterial infections, particularly those caused by Staphylococcus aureus. Cloxacillin and cefazolin are both beta-lactam antibiotics; cloxacillin is an antistaphylococcal penicillin that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), while cefazolin is a first-generation cephalosporin with similar action but broader activity against some Gram-positive organisms. Daptomycin is a lipopeptide antibiotic that disrupts bacterial cell membrane potential, leading to rapid depolarization and cell death. The rationale for combining these agents lies in their complementary mechanisms of action and potential synergistic effects against resistant staphylococci (including MRSA), as well as the prevention of resistance development during therapy[2][3][6]. This combination has been studied primarily in the context of complicated bacteremia or endocarditis due to Staphylococcus aureus.
