Overview

Clozapine + amisulpride is a combination of two atypical antipsychotic drugs used primarily in the management of treatment-resistant schizophrenia. Clozapine acts on multiple neurotransmitter receptors, including dopamine D2, serotonin 5-HT2A, and others, providing broad-spectrum antipsychotic effects. Amisulpride predominantly binds with high affinity to dopamine D2 and D3 receptors as an antagonist. The combination is considered when monotherapy with clozapine does not provide sufficient symptom control; amisulpride is added to enhance dopaminergic blockade and potentially improve both positive and negative symptoms in refractory cases. This strategy may allow for lower doses of clozapine, reducing its side effect burden. However, the benefit over monotherapy remains uncertain based on current evidence, and there are increased risks for adverse effects such as cardiac arrhythmias[1][4][5][8].
