Overview
**Clozapine + atomoxetine** is a combination of two pharmaceutical agents: clozapine, an atypical antipsychotic primarily used for treatment-resistant schizophrenia, and atomoxetine, a selective norepinephrine reuptake inhibitor indicated for attention-deficit/hyperactivity disorder (ADHD). Clozapine acts mainly as an antagonist at dopamine receptor D2 and serotonin receptor 5-HT2A, with additional antagonism at adrenergic, cholinergic, histaminergic, and other dopaminergic/serotonergic receptors. Atomoxetine selectively inhibits the presynaptic norepinephrine transporter to increase synaptic norepinephrine levels. The combination is not standard therapy but may be considered in rare clinical scenarios such as comorbid ADHD in patients with schizophrenia. However, this combination carries a significant risk of QT/QTc prolongation and potentially life-threatening arrhythmias such as torsade de pointes; therefore it should only be used under close medical supervision[1][2][3][4][5].
