CMP-001 + INCAGN01949 is an investigational combination immunotherapy regimen evaluated for the treatment of advanced solid tumors, including stage IV pancreatic adenocarcinoma. CMP-001 (vidutolimod) consists of a CpG-A oligodeoxynucleotide (a TLR9 agonist) encapsulated within a virus-like particle (VLP) derived from the bacteriophage Qβ. It is designed to activate plasmacytoid dendritic cells, triggering type I interferon production and priming a systemic anti-tumor T-cell response. INCAGN01949 is a fully human IgG1 agonistic monoclonal antibody that targets OX40 (CD134), a co-stimulatory receptor expressed on activated T cells. By providing a potent co-stimulatory signal, INCAGN01949 enhances the proliferation, survival, and effector function of T cells primed by the TLR9 agonist. The combination was primarily studied via intratumoral injection to induce an "in situ vaccine" effect, aiming to turn "cold" tumors "hot" and overcome resistance to other immunotherapies.
