CMV-specific donor lymphocyte infusion (DLI) is an investigational adoptive cellular immunotherapy developed by Nelson Chao and colleagues at Duke University. It is designed to restore CMV-specific cellular immunity in patients who have undergone nonmyeloablative allogeneic stem cell transplantation (SCT). The therapy involves the infusion of T cells derived from 3-6/6 HLA-matched family donors that are specifically reactive to cytomegalovirus (CMV) antigens. By providing these functional cytotoxic T lymphocytes (CTLs), the treatment aims to prevent or treat CMV reactivation and disease, which are major causes of morbidity in immunocompromised SCT recipients, without the myelosuppressive side effects associated with standard antiviral pharmacotherapy like ganciclovir.
