Overview

The combination of codeine and metformin represents a potential drug interaction rather than a single formulated product. These two medications are prescribed separately but may be used concomitantly by patients. Metformin is a biguanide antihyperglycemic medication primarily used to treat type 2 diabetes, while codeine is an opioid analgesic used for pain management. When used together, these medications can interact at the transporter level, specifically affecting the organic cation transporter 1 (OCT1). ## Interaction Mechanism The interaction between codeine and metformin occurs because both drugs are affected by the OCT1 transporter. Metformin is a substrate for OCT1, which means it relies on this transporter to enter cells. Codeine and its metabolite morphine are inhibitors of OCT1, with morphine also being a substrate. This interaction at the transporter level can affect the pharmacokinetics of both medications. ## Clinical Implications Epidemiological studies have indicated that concomitant use of codeine and metformin increases the risk of metformin-related gastrointestinal intolerance. This may lead to early discontinuation of metformin treatment within 6 months after the first prescription. A pharmacokinetic study investigating this interaction found that metformin increases morphine and metabolite levels. This bidirectional interaction suggests that not only does codeine affect metformin's effects, but metformin may also influence codeine's metabolism and efficacy. ## Recommendations Due to the potential for increased adverse effects, particularly gastrointestinal intolerance, caution is advised when using codeine and metformin concomitantly. While it's not generally recommended to completely avoid this drug combination, patients should be monitored for increased side effects. Healthcare providers should be aware of this interaction when prescribing these medications together and may need to adjust dosages or consider alternative pain management options for patients on metformin therapy.
