Overview

CODOX-M + IVAC is a high-intensity, alternating chemotherapy regimen primarily utilized for the treatment of Burkitt lymphoma and other high-grade B-cell lymphomas. Developed by Ian Magrath at the National Cancer Institute (NCI), the protocol consists of two distinct blocks: CODOX-M (cyclophosphamide, vincristine, doxorubicin, and high-dose methotrexate) and IVAC (ifosfamide, etoposide, and high-dose cytarabine). The regimen exerts its cytotoxic effects through multiple pathways, including DNA alkylation, inhibition of DNA synthesis via antimetabolite action, and interference with topoisomerase II and microtubule assembly. While highly effective with high cure rates in aggressive lymphomas, it is associated with significant systemic toxicity, particularly myelosuppression and mucositis.
