Overview

Colchicine + methotrexate is a combination of two small molecule drugs, each with distinct mechanisms of action, sometimes used together in clinical practice for their anti-inflammatory and immunomodulatory effects. Colchicine disrupts microtubule polymerization by binding to tubulin, thereby inhibiting neutrophil motility and activity, which reduces inflammation. Methotrexate acts primarily as an antimetabolite by inhibiting dihydrofolate reductase (DHFR), leading to suppression of DNA synthesis and cell proliferation; it also exerts immunosuppressive effects through increased adenosine signaling. Both agents are individually approved for conditions such as gout (colchicine) and rheumatoid arthritis (methotrexate), but may be co-administered off-label in certain inflammatory or autoimmune diseases where dual modulation of leukocyte function is desired[1][4]. The combination has been studied for its ability to reduce leukocyte adhesion and emigration in response to inflammatory mediators[1]. However, the use of both drugs together requires careful monitoring due to potential additive toxicity affecting the kidneys, liver, or blood cells[4].
