Colchicine + ocrelizumab is a combination therapy consisting of colchicine, a small molecule anti-inflammatory agent, and ocrelizumab, a humanized monoclonal antibody targeting CD20 on B cells. Colchicine disrupts microtubule polymerization, inhibiting neutrophil activity and reducing inflammation. Ocrelizumab selectively depletes CD20-positive B cells via antibody-dependent cellular cytotoxicity and complement-mediated lysis. This combination has been studied in patients with multiple sclerosis (MS) who also have familial Mediterranean fever (FMF), particularly those with colchicine-resistant FMF. Clinical evidence suggests that the addition of ocrelizumab to ongoing colchicine therapy significantly reduces the frequency of FMF attacks and joint symptoms in MS+FMF patients[1][3]. The combination leverages both immunomodulatory (ocrelizumab) and anti-inflammatory (colchicine) mechanisms.
