Overview

Cold-stored platelets (CSPs) are a biological blood product consisting of human apheresis platelets stored at 4°C, currently being developed and investigated by Bloodworks Northwest for the reversal of platelet dysfunction and the management of hemorrhage. Unlike conventional platelets stored at room temperature (22°C), CSPs maintain superior hemostatic function, characterized by enhanced thrombin generation and more rapid activation of integrin receptors (Integrin alpha-IIb/beta-3). These properties make them particularly suitable for reversing the effects of dual antiplatelet therapy (DAPT), such as aspirin and P2Y12 inhibitors, in patients experiencing acute bleeding. While CSPs are FDA-approved for use in actively bleeding patients when room temperature-stored platelets are unavailable, Bloodworks is conducting clinical trials (e.g., NCT05111102) to specifically evaluate their efficacy in DAPT reversal.
