colesevelam + sitagliptin

Colesevelam + sitagliptin is a fixed-dose combination (if co-formulated) or co-administration regimen involving two oral small molecule drugs: colesevelam, a bile acid sequestrant used to lower LDL cholesterol and improve glycemic control in type 2 diabetes; and sitagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor used as an oral antihyperglycemic for type 2 diabetes. Colesevelam binds bile acids in the intestine, increasing fecal excretion and leading to upregulation of LDL receptors, while also having glucose-lowering effects through mechanisms likely involving reduced bile acid-mediated signaling. Sitagliptin inhibits DPP-4, prolonging the action of incretins (GLP-1 and GIP), increasing insulin secretion and decreasing glucagon in a glucose-dependent manner. Clinical studies show that their combination leads to greater improvements in glycemic control than either drug alone in patients with type 2 diabetes[4][7]. There is no proprietary, fixed-dose formulation under a unique brand name documented.
