This conditioning and graft-versus-host disease (GVHD) prophylaxis regimen is a multi-drug protocol developed by Ann & Robert H Lurie Children's Hospital of Chicago for pediatric patients undergoing haplo-identical stem cell transplantation (SCT). The protocol is designed for high-risk hematologic malignancies, including leukemia, lymphoma, and myelodysplastic syndromes. It involves a stratified approach to conditioning using agents such as clofarabine, melphalan, thiotepa, busulfan, fludarabine, and total body irradiation (TBI) to achieve myeloablation and disease control. A critical component of the regimen is the use of post-transplant cyclophosphamide (PTCy) for in-vivo T-cell depletion to prevent GVHD, supplemented by tacrolimus and mycophenolate mofetil (MMF). This combination therapy aims to facilitate successful engraftment from partially matched donors while minimizing the risks of severe GVHD and transplant-related mortality.
