Overview

This refers to the therapeutic interventions evaluated in a clinical trial conducted by the University Medical Centre Ljubljana for the treatment of mild to moderate carpal tunnel syndrome (CTS). The study compares ultrasound-guided perineural injections of 5% dextrose water (D5W) and local corticosteroids against surgical release. The corticosteroid component (typically methylprednisolone or betamethasone) acts as a potent anti-inflammatory agent to reduce perineural edema and pressure on the median nerve within the carpal tunnel. The 5% dextrose arm involves perineural injection, which is hypothesized to alleviate neuropathic pain and neurogenic inflammation through osmotic mechanisms or by potentially downregulating pain-sensing receptors on the nerve. The trial aims to demonstrate the non-inferiority of dextrose injections compared to corticosteroids at 12 months and compared to surgical release at 6 months.
