Overview

This combination therapy, consisting of a corticosteroid and a local anesthetic, is being investigated by Wake Forest University Health Sciences for the treatment of chronic plantar fasciitis. The intervention is administered via a specialized percutaneous method known as the infracalcaneal peppering technique. This technique involves a single skin entry point through which the needle is redirected 20-25 times to fenestrate the pathologic plantar fascia while simultaneously depositing the medication. The corticosteroid component is intended to provide anti-inflammatory effects and long-term symptom relief, while the local anesthetic provides immediate analgesia and facilitates the mechanical peppering procedure. The therapy is currently being evaluated in a Phase 1 randomized clinical trial (NCT04443400) to compare the efficacy of the corticosteroid component against mechanical peppering with saline alone.
