Corticosteroids + colchicine is a combination therapy investigated by Mansoura University for the treatment of post-COVID-19 pulmonary fibrosis. This regimen typically involves the administration of a corticosteroid, such as prednisolone (at doses of 20 mg or higher), in conjunction with colchicine (1 mg/day). Corticosteroids act as potent anti-inflammatory agents by binding to and activating glucocorticoid receptors, thereby modulating gene expression to reduce the production of pro-inflammatory cytokines. Colchicine is a microtubule-disrupting agent that binds to tubulin, inhibiting its polymerization and interfering with various cellular processes, including leukocyte chemotaxis and the activation of the NLRP3 inflammasome. The combination aims to leverage both anti-inflammatory and potential anti-fibrotic mechanisms to improve radiological and functional outcomes in patients with persistent lung injury following COVID-19 infection.
