Corticosteroids + intravenous immunoglobulin is a combination therapy involving the concurrent administration of systemic corticosteroids and intravenous immunoglobulin (IVIg). This regimen is used in several immune-mediated and inflammatory conditions, including relapsed immune thrombocytopenia (ITP), Stevens–Johnson syndrome (SJS), toxic epidermal necrolysis (TEN), Kawasaki disease, and chronic inflammatory demyelinating polyradiculoneuropathy (CIDP)[1][2][3][4][6]. **Mechanism of Action:** - Corticosteroids are synthetic analogs of endogenous glucocorticoids that exert potent anti-inflammatory and immunosuppressive effects by modulating gene expression, inhibiting pro-inflammatory cytokine production, suppressing leukocyte migration, and reducing immune cell activation. - Intravenous immunoglobulin consists of pooled IgG antibodies from healthy donors. It modulates the immune response through multiple mechanisms including Fc receptor blockade on phagocytes, neutralization of autoantibodies or pathogenic antigens, inhibition of complement activation, modulation of B-cell function, and suppression of inflammatory cytokines. **Clinical Use:** Combination therapy has been shown to provide more rapid clinical improvement than monotherapy with either agent alone in relapsed ITP[3][5], SJS/TEN[1][6][7], CIDP[2], and high-risk Kawasaki disease[4]. The combination may reduce mortality rates in SJS/TEN patients compared to monotherapy or supportive care alone. In ITP, it leads to faster platelet recovery; in Kawasaki disease it reduces coronary artery complications; in CIDP it induces remission more rapidly than steroids alone.
