Overview

Course A2 + Vin is a specific multi-agent chemotherapy block utilized within a modified ALCL99 regimen for the treatment of pediatric and adolescent patients with newly diagnosed high-risk anaplastic large cell lymphoma (ALCL). Developed by the Children's Cancer Group, China (CCCG) for the CCCG-ALCL-2020 phase 3 clinical trial, this course is an adaptation of the NHL-BFM90 backbone. The Course A2 component typically consists of dexamethasone, methotrexate, ifosfamide, cytarabine, and etoposide. The Vin designation indicates the inclusion of vinorelbine, a semi-synthetic vinca alkaloid that serves as a substitute for vinblastine, which has faced availability issues in China. Vinorelbine exerts its cytotoxic effect by binding to tubulin and inhibiting microtubule polymerization, thereby inducing cell cycle arrest in mitosis. This combination regimen aims to improve outcomes in ALK-positive and other high-risk ALCL subtypes by leveraging the synergistic effects of antimetabolites, alkylating agents, and microtubule inhibitors.
