This is a combination drug consisting of a cyclooxygenase (COX) inhibitor and nitroglycerin. COX inhibitors, which include both non-selective NSAIDs and selective COX-2 inhibitors, act by inhibiting the cyclooxygenase enzymes responsible for converting arachidonic acid to prostaglandins and thromboxanes, thereby reducing inflammation, pain, and fever[4][5]. Nitroglycerin is an organic nitrate that acts as a nitric oxide donor; it causes vasodilation by releasing nitric oxide (NO), which activates guanylate cyclase in vascular smooth muscle cells leading to increased cyclic GMP and subsequent relaxation of blood vessels. The combination may be considered for conditions where both anti-inflammatory effects (from the COX inhibitor) and vasodilatory/anti-anginal effects (from nitroglycerin) are desired. There is evidence of molecular cross-talk between NO donors like nitroglycerin and the prostaglandin pathway modulated by COX enzymes[2]. This interaction can influence headache pathogenesis or cardiovascular risk profiles depending on the specific agents used[1][3].
